Stir together sugar, cocoa, cornstarch, and salt in a saucepan. Place over medium heat, and stir in milk. Cook over low heat, stirring constantly, until the mixture thickens enough to coat the back of a spoon or spatula. Remove from heat, and stir in butter and vanilla. Distribute among 4 small cups or ramekins.
Serve warm, or chill in the refrigerator until serving. Optional: Top with whipped cream and shaved chocolate.
